State disciplines health care providers

OLYMPIA¾ The Washington State Department of Health has taken disciplinary actions or withdrawn charges against health care providers in our state.

The department’s Health Systems Quality Assurance Division works with boards, commissions, and advisory committees to set licensing standards for more than 70 health care professions (e.g., medical doctors, nurses, counselors).

Information about health care providers is on the agency website. Click on “Look up a healthcare provider license” in the “How Do I?” section of the Department of Health (doh.wa.gov). The site includes information about a health care provider’s license status, the expiration and renewal date of their credential, disciplinary actions and copies of legal documents issued after July 1998. This information is also available by calling 360-236-4700. Consumers who think a health care provider acted unprofessionally are also encouraged to call and report their complaint.

Benton County

In May 2012 the Mental Health Counselor Program dismissed the statement of charges against Hector Roman (MC60159695).

Clallam County

In May 2012 the Chemical Dependency Professional Program granted the credential of chemical dependency professional Stephanie Marie Trumbull (CP60253597) and placed it on probation for three years.

Clark County

In May 2012 the East Asian Medicine Practitioner Program dismissed the May 2011 statement of charges against Susan Kay Shultz (AC00000209).

Island County

In May 2012 the Chemical Dependency Professional Program granted the credential of chemical dependency professional trainee Robert Wesley Powell (CO60283753) and placed it on probation for two years.

King County

In May 2012 the Nursing Assistant Program denied the certified nursing assistant application of Priscilla Ann Arizmendiz (NC60228893). Arizmendiz was convicted of third-degree theft on three separate occasions and failed to include this information on her application.

In May 2012 the Agency Affiliated Counselor Program granted the credential of agency affiliated counselor Lori Luan Hendon-Coverson (CG60257803) and placed it on probation for one year.

In April 2012 the Health Care Assistant Program denied the health care assistant application of Christina Lynn Hallagin (HC60129514). Hallagin doesn’t meet the criteria for a health care assistant in Washington.

In May 2012 the Nursing Assistant Program granted the credential of certified nursing assistant Max Cong Le (NC60281681) and placed it on probation for two years.

In May 2012 the Pharmacy Board charged pharmacist Patrick G. Oyco (PH00021211) with unprofessional conduct. While Oyco was the responsible pharmacy manager of Cherry Street.

Pharmacy, he allegedly failed to keep complete and current records. The pharmacy he operated didn’t meet the board’s requirements.

Kitsap County

In May 2012 the Nursing Assistant Program reinstated the credential of certified nursing assistant Roxanne D. Warren (NC10019926) and placed her credential on probation for two years.

Kittitas County

In June 2012 the Nursing Commission charged registered nurse Nancy A. Grossman (RN00078146) with failing to comply with a previous stipulation.

Klickitat County

In May 2012 the Nursing Commission charged registered nurse Peggy Jo Lynch (RN60235970) with unprofessional conduct. Lynch allegedly tested positive for marijuana on a pre-employment drug screen.

Lewis County

In May 2012 the Nursing Commission charged registered nurse Susan Marie Taylor (RN00099538) with unprofessional conduct for failing to comply with a previous stipulation.

Mason County

In May 2012 the Chemical Dependency Professional Program granted the credential of chemical dependency professional trainee Grant M. Butler (CO60214074) and placed it on probation for  three years.

Pierce County

In May 2012 the Medical Commission charged physician Kimberly M. Chan (MD00046109) with unprofessional conduct for failing to comply with a previous stipulation.

In May 2012 the Chemical Dependency Professional Program ended the probation agreement against chemical dependency professional Joy Ann Herman (CP60145240).

In May 2012 the Nursing Assistant Program reinstated the credential of certified nursing assistant Milton K. Nichols, Jr. (NC10021890).

In May 2012 the Nursing Commission charged registered nurse Charity A. Ridgley (RN00176856) with unprofessional conduct for failing to comply with a previous stipulation.

In May 2012 the Dental Commission ended the probation order against dentist Donald Russell Swanson (DE00004550).

Skagit County

In May 2012 the Chemical Dependency Professional Program charged chemical dependency professional trainee Tonya Leeann Pearson (CO60150076) with unprofessional conduct for failing to comply with a previous stipulation.

Snohomish County

In May 2012 the Health Care Assistant Program granted the credential of health care assistant Ashley Danielle Foland (HC60239203) and placed it on probation for one year.

Spokane County

In May 2012 the Chiropractic Commission charged chiropractor Chad W. Bess (CH00034347) with unprofessional conduct. Bess was allegedly arrested in 2011 for possession of methamphetamine, and admitted to the arresting officer that he was addicted to Oxycontin. A commission investigator mailed a letter to Bess asking about the incident, and he failed to respond.

Out of state

Arizona: In May 2012 the Nursing Commission placed the license of registered nurse Lisa K. Scarbeary (RN60083034) on probation for at least 36 months.

California: In June 2012 the Veterinary Board of Governors charged veterinarian Herbert L. Ho (VT00002317) with unprofessional conduct. Ho’s license to practice veterinary medicine in California was revoked, and he voluntarily surrendered his veterinary medicine license in British Columbia.

Note to Editors: Health care providers charged with unprofessional conduct have 20 days to respond to the Department of Health in writing. The case then enters the settlement process. If no disciplinary agreement can be reached, the case will go to a hearing.
