State revokes, suspends licenses, certifications, registrations of health care providers
OLYMPIA¾ The Washington State Department of Health has revoked or suspended the licenses, certifications, or registrations of health care providers in our state. The department has also immediately suspended the credentials of people who have been prohibited from practicing in other states.
The department’s Health Systems Quality Assurance Division works with boards, commissions and advisory committees to set licensing standards for more than 70 health care professions (e.g., medical doctors, nurses, counselors).
Information about health care providers is on the agency’s website. Click on “Look up a healthcare provider license” in the “How Do I?” section of the Department of Health home page (www.doh.wa.gov). The site includes information about a health care provider’s license status, the expiration and renewal date of their credential, disciplinary actions and copies of legal documents issued after July 1998. This information is also available by calling 360-236-4700. Consumers who think a health care provider acted unprofessionally are also encouraged to call and report their complaint.
Clallam County
In July 2012 the Department of Health suspended the credentials of certified nursing assistant Katelyn Mary Williams (NC60189305) (NA60141880) for at least two years. Williams tested positive for amphetamine and methamphetamine.
Clark County
In July 2012 the Department of Health issued a notice of intent to issue a cease and desist order to Benjamin Jeanotte, for providing massage services without a license.
In July 2012 the Nursing Commission suspended the credential of registered nurse Linda C. Weinzimmer-Kirk (RN00084119) after she failed to respond to charges that she submitted false records of providing home care services when she did not provide the services.
In July 2012 the Department of Health issued an intent to issue a cease and desist order to Douglas K. Dobberfuhl. Previously Dobberfuhl applied for a Washington counseling credential but was denied because he had surrendered his counseling credential in Wisconsin following charges of inappropriate sexual conduct with a client. In 2011, Dobberfuhl provided counseling services in Washington without a license.
In July the Department of Health suspended the credential of certified nursing assistant Jennifer Dawn McCann (NC10017642) for at least five years. McCann diverted patient medications for her personal use.
Cowlitz County
In July 2012 the Department of Health suspended the credential of health care assistant Virginia M. Knight (HC60211149). Knight was asked by her employer to submit a urine sample for drug testing; instead she submitted the urine of a patient. She admitted to using marijuana and taking unprescribed medication.
Grays Harbor
In July 2012 the Department of Health permanently revoked the credential of agency affiliated counselor Vernon C. Armstrong (CG60155940). A sexual protection order was entered against Armstrong for sexually assaulting a child who had previously been his patient.
Kittitas County
In July 2012 the Nursing Commission suspended the credential of registered nurse Nancy A. Grossman (RN00078146). Grossman did not fulfill a previous agreement to complete a substance abuse monitoring program. Grossman tested positive for a controlled substance.
King County
In July 2012 the Board of Pharmacy suspended the credential of pharmacy technician Mesfin A. Gedamu (VA60067079)(VB00070803) after Gedamu gave controlled substances to a person who did not have a prescription.
In July 2012 the Department of Health suspended the credential of agency affiliated counselor Kim Hunter Norward Sr. (CG60145889) for at least five years. Norward had a sexual relationship with a patient.
In July 2012 the Veterinary Board suspended the credential of veterinary technician Aaron Todd Scofield (AT60237131) for at least two years. Scofield diverted a controlled substance for personal use and altered medical records.
Kitsap County
In July 2012 the Department of Health suspended the credential of registered nurse assistant Jessica Nicol Cody (NA60061582) for 6 months. If Cody completes agreed requirements, she may apply for credential reinstatement, and will be placed on probation for at least 18 months from the date her license is reinstated. Cody slapped a vulnerable adult client.
Mason County
In July 2012 the Department of Health suspended the credential of chemical dependency professional trainee Philip Soaring Hawk Peterson (CO60214535) after he failed to meet the conditions of an order to participate in a substance abuse monitoring program.
Okanogan
In July the Department of Health suspended the credentials of x-ray technician Travis J. McFarland (XT00006387)(RT60056326). McFarland forged a prescription for a controlled substance. He later failed to comply with a substance abuse monitoring program.
Pierce County
In July 2012 the Department of Health suspended the credential of registered nursing assistant Lynniah Grayson (NA60226177)(NC60239688) for at least five years. Grayson was convicted of third-degree assault and first-degree theft. Her license was expired but was eligible for renewal.
In July 2012 the Department of Health suspended the credential of registered nursing assistant Tina Anne Grove (NA60230681) for at least five years. While employed as the manager of a facility providing in-home oversight for vulnerable patients, Grove failed to protect a patient’s safety.
In July 2012 the Department of Health suspended the credential of registered nursing assistant Jessica A. Shoaf (NA60019424) for at least five years. Shoaf mistreated a vulnerable adult in her care and did not report physical injuries she inflicted on the client.
In July 2012 the Department of Health issued a cease and desist order against Tanya Hunter. Hunter advertised and operated a business selling liquid hCG and hypodermic needles for the purpose of weight loss. Hunter does not have a credential to provide any type of medical service in Washington.
In July 2012 the Department of Health suspended the credential of agency affiliated counselor Melissa Ann Marie Wright (CG60109517) after she failed to cooperate with an investigation.
In July 2012 the Department of Health suspended the credential of massage practitioner Artyom Nicholas Davenport (MA60112076). Davenport did not meet the terms of a previous agreement requiring compliance with a substance abuse monitoring program.
In July 2012 the Department of Health suspended the credential of certified nursing assistant Raul Sosa Corona (NC60143696) for at least three years. Corona failed to report an injury that occurred to a patient under his care.
In July 2012 the Department of Health suspended the credential of agency affiliated counselor and registered nursing assistant April Ann Stewart (CG60187202)(NA00191503). Stewart tested positive for methamphetamine and marijuana and neglected a vulnerable adult under her care.
In July 2012 the Department of Health suspended the credential of registered nursing assistant Peter Gichimu Kimani (NA60167029) for at least three years. Kimani neglected a vulnerable adult under his care.
Skagit County
In July 2012 the Department of Health suspended the credential of chemical dependency professional trainee Larry Gene Howard (CO60194291) until he can complete the terms set in a previous order.
Snohomish County
In July 2012 the Chiropractic Commission suspended the credential of chiropractor Alex W. Styles (CH00002335) for at least ten years. Styles was convicted of second-degree theft and making false statements. Styles billed the state for services he did not provide.
In July 2012 the Board of Pharmacy suspended the credential of pharmacist Mary Jo Davenport (VA00018983). Davenport violated a previous order to participate in a substance abuse monitoring program.
Spokane County
In July 2012 the Department of Health suspended the credential of certified nursing assistant Terra L. Callaghan (NC10058912) for at least five years. Callaghan took a check from a patient and her sister attempted to cash it.
Thurston County
In July 2012 the Occupational Therapy Practice Board suspended the credential of occupational therapy assistant Sandra M. Severin (OC60002595). Severin is prohibited from practicing as an occupational therapy assistant in California for failing to comply with terms related to a testing program.
Walla Walla
In July 2012 the Department of Health suspended the credential of respiratory care practitioner Stephen Douglas Miller (LR60076415). Miller was convicted of third-degree assault.
Whatcom
In July 2012 the Department of Health suspended the credential of health care assistant Tania A. Swanberg (HC00133167) after Swanberg admitted to calling in and filling unauthorized prescriptions for her personal use.
Yakima County
In July 2012 the Department of Health suspended the credential of chemical dependency professional trainee Frances Salinas Gonzalez (CO60256819). Gonzalez did not meet the conditions of an agreement to participate in a substance abuse monitoring program.
Out of State
Colorado
In July 2012 the Medical Commission suspended the credential of physician Charlotte E. Hovey (MD00036921). Hovey agreed not to practice medicine in Colorado after the state concluded she could not safely practice medicine.
