OLYMPIA, Washington – The Washington State Department of Health has revoked or suspended the licenses, certifications, or registrations of health care providers in our state.

The department has also immediately suspended the credentials of people who have been prohibited from practicing in other states.

The department’s Health Systems Quality Assurance Division works with boards, commissions and advisory committees to set licensing standards for more than 80 health care professions, [i.e. MDs, Nurses, Counselors].  Information about health care providers is on the agency’s website.

Click on “Look up a healthcare provider license” in the “How Do I?” section of the Department of Health home page (doh.wa.gov). The site includes information about a health care provider’s license status, the expiration and renewal date of their credential, disciplinary actions and copies of legal documents issued after July 1998.

Consumers who think a health care provider acted nprofessionally are also encouraged to call and report their complaint.

BENTON COUNTY

In November 2013, the Office of the Secretary suspended the credential of CNA Eva Barany (NC10020123) for five years. Barany borrowed $2,000 from a patient to cover moving expenses and never paid it back.

CLALLAM COUNTY

In September 2013 the Department of Health revoked the credential of RNA Laurie M. Morehead (NA00159879). She tended to a homebound client’s fractured arm and didn’t take him to a hospital, practicing outside the scope of her field.  The injury became infected and the client died. She also was intoxicated while tending to the client and took cash gifts from him.

In November 2013 the Board of Naturopathy suspended the credential of Naturopathic Physician Richard A. Marschall (NT00000532) for at least a year. He allegedly prescribed an unapproved drug from China to about 170 patients to help them lose weight.

CLARK COUNTY

In November 2013 the secretary of health indefinitely suspended the credential of Paramedic Sarah Roskam (ES01170784). She relapsed and was diagnosed with opioid dependence after completing a substance abuse program.

In November 2013 the Nursing Care Quality Assurance Commission suspended the credential of LPN Paula Ann Harris (LP00040089) for at least three years. Harris was deemed unable to practice with reasonable skill and safety. She was hospitalized in a psychiatric unit after having suicidal thoughts.

COWLITZ COUNTY

In November 2013 the Examining Board of Psychology accepted the surrendered credential of Psychologist Karen Ann Pauly (PY00002823). She became involved in a sexual relationship
with a patient and took $15,000 from the patient for personal items, spending cash, and to help avoid foreclosure on her home. Pauly also bought prepaid cell phones for the patient, so his wife
wouldn’t find out about their relationship.

FRANKLIN COUNTY

In November 2013 the Nursing Care Quality Assurance Commission indefinitely suspended the credential of RN Kylee L. Rainwater (RN00166118). She didn’t comply with the
terms of her drug monitoring contract, testing positive for tramadol.

KING COUNTY

In November 2013 the secretary of health withdrew the statement of charges against NA Wondu G. Tanetse (NA60187481 and NC60190437).

In November 2013 the Board of Naturopathy indefinitely suspended the credential of Naturopathic Physician Anastasia D. Jones (NT00001612). She didn’t give a patient their medical records or itemized billing and didn’t cooperate with an investigation by the department.

In November 2013 the secretary of health suspended the credential of RNA Regina Daniels (NA00139243) for at least five years. Daniels was convicted of second-degree criminal mistreatment and found to have neglected vulnerable adults while working at an adult family home.

In November 2013 the Department of Health indefinitely suspended the credential of Practical Nurse Wendy Frauenberg (LP00045192). She didn’t comply with her reinstatement agreement by not completing a commission-approved refresher course.

In November 2013 the Medical Quality Assurance Commission accepted the surrender of the credential of Physician and Surgeon William L. Lanzer (MD00022061). He allegedly relapsed for a fifth time, violating the terms of his monitoring contract.

In November 2013 the Nursing Care Quality Assurance Commission suspended the credential of RN Patricia Marie Bursch (RN00058788) for at least a year. She’s charged with
exploiting a vulnerable adult after writing and cashing several checks from a trust account for her mother, who suffers from dementia.

In October 2013 the secretary of health suspended the credential of CNA Lucia Lacsamana Washington (NC10001426) for at least two years. Washington neglected a
vulnerable adult in her care.

In November 2013 the secretary of health suspended the credential of RNA Elena A. Dilla (NA60298768) for at least two years. Dilla neglected a vulnerable adult while she was employed as a caregiver at an adult family home.

KITSAP COUNTY

In November 2013 the secretary of health suspended the credential of RNA KC Marie Betancur (NA60283644) for at least five years. Betancur stole $100 from a patient at an assisted living community.

PIERCE COUNTY

In November 2013 the Nursing Care Quality Assurance Commission suspended the credential of
RN Lisa Anne Daily (RN00110540)  for at least two years. Daily admitted to having a substance abuse problem and attempting to take Fentanyl from her employer for personal use.

In November 2013 the secretary of health suspended the credential of CNA Kathleen A. Sallade (NC10102214) for at least 10 years. Sallade was convicted of first-degree trafficking stolen property. While working at an assisted living facility, she stole a resident’s jewelry and sold it at a pawn shop.

In November 2013 the Nursing Care Quality Assurance Commission suspended the credential of LPN Diana L. Snead (LP00025789) for at least two years. Snead didn’t enter
a treatment program, failing to meet the terms of her substance abuse contract.

In November 2013 the secretary of health suspended the credential of RNA Suia Seufale (NA60269598) for at least five years. Seufale was accused of stealing money and valuables from an assisted living facility. During the investigation, it was discovered that Seufale and her mother had been charged with second-degree theft. Criminal charging documents alleged that they were depositing fraudulent checks into each of their bank accounts. Seufale failed to cooperate with the Department of Health investigation.

SNOHOMISH COUNTY

In October 2013 the Department of Health indefinitely suspended the credential of CNA Leandra Marie Lynch (NC60135021). She didn’t submit employer reports or
reports of unemployment every three months, failing to meet the terms of her reinstatement order.

In November 2013 the Pharmacy Quality Assurance Commission indefinitely suspended the credentials of Pharmacy Assistant and Pharmacy Technician Ramsey Bruce Leggett (VB60115413 and VA60167258). He stole four tablets of diazepam for his personal use from his former employer.

In October 2013 the secretary of health denied reactivating the expired credential of CNA Kristen Marie Carter (NC10055741). Her credential was suspended by the Oregon State Board of Nursing for not cooperating with its investigation.

SPOKANE COUNTY

In October 2013 the Department of Health indefinitely suspended the credentials of RCNA, CNA Kristy L. Tooley Willcockson (NA60126326 and NC60163093). She didn’t meet the terms of her reinstatement order by not submitting her quarterly reports.

In November 2013 the Nursing Care Quality Assurance Commission indefinitely suspended the credential of LPN Gail L. Hunt (LP00038766). Hunt allegedly pressured a nursing assistant to perform a task that was outside the scope of her practice. She’s also charged with improperly administering glucose to a patient and changing the records to hide the mistake.

In November 2013 the Nursing Care Quality Assurance Commission indefinitely suspended the credential of RN Linda M. Gusch (RN00096929). She allegedly neglected patients on several occasions. In one incident, Gusch reported that a patient at a long-term care facility didn’t need immediate help, yet the patient was coughing uncontrollably and received treatment from another staff member.

In November 2013 the secretary of health suspended the credential of Social Worker Dennis J. Vandermause (LW00009372) for at least five years. He was indicted for billing Medicaid services he didn’t perform, and convicted of making false statements in a federal health care program.

THURSTON COUNTY

In November 2013 the Office of the Secretary suspended the credential of Health Care Assistant Angela Christine Coombes (HT60362044) for five years. She had sexual relationships with two patients.

YAKIMA COUNTY

In November 2013 the secretary of health indefinitely suspended the credential of CNA Valerie Esquer (NC10050018). She tested positive for marijuana and cocaine.

STATE OF OREGON

In November 2013 the Medical Quality Assurance Commission terminated the terms and conditions against Physician and Surgeon Jerrold R. Darm (MD00018472).

In November 2013 the secretary of health issued a permanent cease and desist order to Sherry Dress. She helped deliver babies without a license to practice as a midwife in
Washington.

 In November 2013 the Nursing Care Quality Assurance Commission suspended the credential of RN Leona M. Smith (RN00177534) for 18 months. Smith’s credential was revoked by the Oregon State Board of Nursing for taking and texting a photo of a patient’s bare buttocks without their knowledge or consent.
