7.9M #NepalQuake | The Worst Disaster in Nepalese History in 80 Years

Earthquake

On Saturday April 25th, a magnitude 7.9 earthquake struck the Lamjung District of Nepal, around 1211 NPT [Seattle is 12 hrs 45 mins behind].

21 aftershocks have also struck the region including a 5.2 in the mountains causing a landslide on Mt. Everest.

It was the biggest earthquake to strike the country in 80 years. The earthquake was felt across the region from India to China and left immense destruction, flattening sections of Kathmandu and triggering avalanches in mount Everest.

17 people have been reportedly killed when the face of the mountain slide down on to Camps 1 and 2 below after a 5.2M earthquake [aftershock] triggered a landslide.

Initial government reports confirm that 30 out of 75 districts in the country have been affected in the Western and Central Regions, including Kathmandu Valley districts. This includes mountain and hilly areas, in areas where rural populations are dispersed, as well as some very densely populated districts and Nepal’s two largest cities – greater Kathmandu and Pokhara.  The worst affected districts include Sindulpalchowk, Kavre, Nuwakot, Rasuwa, Dolakha in the CR and Kaski, Gorkha, Lamjung in the western region.

Another Hospira Manufactured Drug Is Being Recalled

FOR IMMEDIATE RELEASE — March 6, 2015 — LAKE FOREST, Ill. — Hospira, Inc., (NYSE: HSP) has announced a voluntary recall of one lot of Magnesium Sulfate in 5% Dextrose, Inj., USP, 10 mg/mL (NDC: 0409-6727-23, Lot 42-120-JT, Expiry 1DEC2015) to the user level due to confirmed customer reports of an incorrect barcode on the primary bag labeling. The barcode on the overwrap is correct; however, there is potential for the primary container barcode to be mislabeled with the barcode for Heparin Sodium 2000 USP units/1000 mL in 0.9% Sodium Chloride Inj. The product is labeled with the correct printed name on the primary container and overwrap.

 If the incorrect barcode on Magnesium Sulfate in 5% Dextrose, Inj., USP, 10 mg/ mL is not detected prior to dispensing or administration to a patient, and the product is administered based on the printed name, patient harm is unlikely since the barcode on the overwrap and readable text on the primary container and overwrap are correct. However, if detected, there is the potential for delay in treatment of magnesium sulfate in 5% dextrose, that can result in life-threatening seizures, stroke, cerebral hemorrhage and maternal death, and attendant risks to the fetus, including fetal demise. Administration of the magnesium sulfate drug product to a patient who is prescribed heparin and in whom the magnesium sulfate is contraindicated can result in serious adverse events related to the drug’s pharmacologic action and may require medical intervention. Although serious in nature, the likelihood of this risk to occur is low due to the high detectability of this nonconformance. To date, Hospira has not received reports of any adverse events associated with this issue for this lot.

Magnesium sulfate in 5% dextrose injection, USP, is a prescription product administered intravenously for the prevention and control of seizures in preeclampsia and eclampsia, respectively. The product is packaged in 50/100 mL container bags and sold 24 bags per carton (NDC: 0409-6727-23, Lot 42-120-JT, Expiry 1DEC2015). The lot was distributed nationwide in the U.S. to wholesalers, distributers and hospitals from October 2014 to January 2015. Hospira has initiated an investigation to determine the root cause and corrective and preventive actions.

Anyone with an existing inventory of the recalled lot should stop use and distribution and quarantine the product immediately. Customers should notify all users in their facility. Customers who have further distributed the recalled product should notify any accounts or additional locations which may have received the recalled product and instruct them if they have redistributed the product to notify their accounts, locations or facilities to the consumer level. Hospira will be notifying its direct customers via a recall letter and is arranging for impacted product to be returned to Stericycle in the United States. Nationwide Drug Recall by Hospira due to Particulate Matter Present

FOR IMMEDIATE RELEASE – March 5, 2015 – Lake Forest, Ill. – Hospira, Inc. (NYSE: HSP), announced today that it will initiate a voluntary nationwide recall of one lot of 0.9% Sodium Chloride Injection, USP, 250 mL VisIV™ flex container (NDC 0409-7983-25, Lot 45-110-C6, Expiry 1MAR2016) to the user level due to one confirmed customer report of particulate in a single unit. The foreign particle was confirmed by Hospira as human hair free-floating within the solution.

To date, Hospira has not received reports of any adverse events associated with this issue for this lot.

In the unlikely event that the particulate breaks and pieces are able to pass through the intravenous catheter, injected particulate material may result in localized inflammation, phlebitis, allergic reaction, granuloma formation or microembolic effects (IV only) and/or low-level allergic response. Capillaries which may be as small as the size of a red blood cell, approximately seven microns in diameter, may become occluded.

Patients with pre-existing condition of trauma or other medical condition that adversely affects the microvascular blood supply are at an increased risk. However, the likelihood of these risks is low as there is no evidence indicating that IV injection of inert particles results in harm to patients when only a small amount over a limited period of time is administered.

Delay of therapy may occur due to observation of particulate at the point of care. However, this delay is likely to be of negligible clinical significance as this medication is administered by a health care provider and remediation is readily available.

This lot was distributed nationwide from December 2014 through January 2015. Hospira has initiated an investigation to determine the root cause and corrective and preventive actions.

MEDICAL DEVICE RECALL: PadPro and R2 Multi-function Defibrillation Electrodes By CONMED

FOR IMMEDIATE RELEASE – November 25, 2014 – Utica, New York – CONMED Corporation announces that it has notified customers of a Voluntary Urgent Device Correction for certain PadPro and R2 Multi-function Defibrillation Electrodes due to a connector compatibility issue with Philips FR3 and FRx Defibrillator Units.

These electrodes will not connect with Philips FR3 or FRx AED units. The FRx AED unit requires the pads to be pre-connected, and will issue a continuous alarm chirp to alert the user that the proper pads are not connected to the unit prior to use. The FR3, however, does not require pre-connection and the user will not discover the incompatibility issue until the AED must be used. This may result in a delay in therapy.

CONMED has not received any complaints or Medwatch reports of injuries associated with this Voluntary Device Correction.

The affected multi-function electrodes were distributed globally to distributors and medical facilities from March 1, 2012 through October 29, 2014.

CONMED alerted customers to this issue by letter on November 6, 2014 and is in the process of revising its labeling to clarify the use of these electrodes as incompatible with the Philips FR3 and FRx AED units. There are a total of 174,610 electrodes affected by this voluntary device correction. No product needs to be returned to CONMED.

FDA ALERT NOTIFICATION: Medical [Neurology] Audience

Tecfidera (dimethyl fumarate) by Biogen Idec: Drug Safety Communication – Case of Rare Brain Infection PML Reported

AUDIENCE: Neurology

ISSUE: FDA is warning that a patient with multiple sclerosis (MS) who was being treated with Tecfidera (dimethyl fumarate) developed a rare and serious brain infection called progressive multifocal leukoencephalopathy (PML), and later died. The patient who died was not taking any other drugs that affect the immune system or drugs that are thought to be associated with PML. As a result, information describing this case of PML is being added to the Tecfidera drug label.

PML is a rare and serious brain infection caused by the John Cunningham (JC) virus. The JC virus is a common virus that is harmless in most people but can cause PML in some patients who have weakened immune systems.

BACKGROUND: Tecfidera is a drug used to treat relapsing forms of multiple sclerosis (MS), a brain and spinal cord disease in which patients experience multiple episodes of weakness, numbness, and other nervous system signs and symptoms that partially or completely resolve overs weeks or months. Patients may develop persistent symptoms and disability over time.

RECOMMENDATION: Healthcare professionals should:

  • Tell patients taking Tecfidera to contact you if they develop any symptoms that may be suggestive of progressive multifocal leukoencephalopathy (PML). Symptoms of PML are diverse, progress over days to weeks, and include the following: progressive weakness on one side of the body or clumsiness of limbs; disturbance of vision; and changes in thinking, memory and orientation, leading to confusion and personality changes. The progression of deficits can lead to severe disability or death.
  • Stop Tecfidera immediately at the first sign or symptom suggestive of PML and perform an appropriate diagnostic evaluation.
  • Monitor lymphocyte counts in Tecfidera-treated patients according to approved labeling.
